Prescription Filling Errors
Filling prescriptions consists of three activities: The Drug Utilization Review (DUR), counting the correct quantity of pills, and individual consultations with each patient. As pharmacies fill more and more prescriptions, many states, as well as the federal government, are seeking to reduce the number of prescription errors and thus require the practice of DURs.
A DUR requires that "patient profiles" must be properly updated for each medication prescribed. Profiles should include relevant patient history, medications in use, known medication allergies, drug interaction information, information on proper dosage, and other pertinent information.
Pharmacists are authorized to perform DURs, whereas technicians are not. Court cases have revealed that some pharmacies, both large and not so large, have found ways to circumvent government regulations or disregard then altogether, eliminating the role of the physician and/or pharmacist, thereby putting patients at risk.
